A major component of mental health is promoting wellness in day to day life. Without proper attention to wellness, a person can feel lost and lack direction in their life. People who struggle with wellness often struggle with anxiety, depressed mood, and trouble with stress management.
Well is not a state of mind, but rather a responsibility to maintain. There are different areas of wellness, each of which require attention and conscious efforts to preserve. Being aware of the attention given to wellness can help clients stay present and aware of their behavior and mental health.
About This Worksheet
This is the Wellness Assessment worksheet. On this worksheet, the client is asked to keep a weekly record of how they tend to their wellness. The Wellness Assessment worksheet helps the client keep track of 6 areas of wellness over the course of 4 weeks. It is a great homework assignment for adolescents and adults in individual counseling.
Have a conversation with the client about their definition of wellness. Allow the client to explain their wellness routine and identify areas that require their attention. After the conversation, assign the client with this homework assignment. Instruct the client to keep a weekly record of their wellness activities over the course of the next four weeks.
Each week, check in with the client about how they are doing with the homework assignment. At the end of the four weeks review the results and help the client establish a wellness routine.
