Social Support Worksheet

A strong social support network is an important component of maintaining mental health. Whether suffering from a mental health condition, recovering from addiction, or just managing daily stressors, social support can be a huge resource for managing mental wellness. Sometimes it can be hard to reach out for support, but helping a client know who is in their support system and why they can trust those people can be helpful for them as they learn how to manage their stress and emotions.

In therapy, clients are encouraged to establish and maintain a social support network. Having a clear idea of who is available to support your client can aid in the therapy process. Friends, family, and loved ones can all be a part of a social support network if they are trustworthy and reliable.

About This Worksheet

This is the Social Support worksheet. On this worksheet, the client is asked to record up to four reliable people in their social support network. Whether the client is able to record one or four people they can depend on, writing down who is in their social support network can be a powerful tool for managing emotions while in therapy.

This worksheet can be an aid in treatment planning and can be used at any point in therapy. It is ideal for adolescents and adults and can be best utilized in individual sessions.

Instructions

Review the importance of a social support network with the client. Explain to them that the number of people in the social support network is not as important as the quality of support they get from each person.

Allow the client space and time to choose up to four people to identify on the Social Support worksheet and to answer the follow-up questions for each person. When the client has completed the worksheet, reflect on why they chose each person and how they are supportive of the client. Provide a copy of the worksheet for the client at the end of the session.
