Change takes time. Change can be challenging for people to accomplish, and it is often something that people resist. Change is scary, but can also be exciting, healthy, and liberating. Some changes cause pain while others cause relief. Whatever the need is for change, it can be difficult for a client to facilitate change in their life.

The stages of change are an important concept to understand when trying to facilitate change or when helping a client make changes and cope with changes they are going through. Understanding the stages of change can help the process of change seem less overwhelming. It provides a plan of action that empowers the client to stay engaged in the changing process.

About This Worksheet

This is the Stages Of Change worksheet. This worksheet outlines each of the 6 stages of change and provides reflection questions for your client to answer. On this worksheet, your client is provided with a means to relate to their experience with change and understand that the challenges they face in each stage are normal.

This worksheet can be great in individual, group, and family counseling settings. It can be helpful for adults and adolescents who are struggling with several different issues and challenges. This worksheet can be particularly helpful for addiction recovery and adjustment issues.

Instructions

This worksheet provides an overview of each of the 6 stages of change. Before the client starts answering questions of the worksheet, review and explain the meaning of each stage. Use examples of what each stage may feel like and relate to how a person may struggle in each stage.

After you have reviewed the stages with your client, instruct them to identify which stage of change they feel they are experiencing. Use your professional insight to help them determine which stage best suits their experience. When your client has identified their stage of change, give them time to complete the reflection questions in the stage’s section.

This worksheet may be used as a working document. The reflection questions can be answered as the client enters a new stage. This worksheet can be especially useful to aid in treatment planning.
