Goal Exploration Worksheet

Goal setting can be a great method of promoting good mental health. When people set goals for themselves, they set themselves on the path for success. While that path can be challenging at times, goals can give people a sense of purpose and value.

A great way to help people who are struggling with their mental health is to encourage them to set realistic goals for themselves. Whether it is for career, lifestyle, family or a hobby, setting goals can help people internalize accomplishments. Internalizing accomplishments improves self-esteem and confidence.

About This Worksheet

This is the Goal Exploration worksheet. On this worksheet, a client can brainstorm potential goals they would like to set for themselves. The worksheet provides brainstorming questions to consider when setting goals. It also helps the client set a realistic timeline.

This worksheet can work great for adolescents and adults. It can be utilized in individual, couples, family and group counseling.

Instructions

Open a conversation with the client(s) about what they would like their future to look like. Explain the importance and value behind setting goals, and how goals can benefit their mental health. Then, allow the client time to determine their desired goal and answer the reflection questions on the worksheet.

When the client(s) have completed the worksheet, invite them to share their goal and repossess on the reflection questions. Encourage them to begin the process of pursuing their goals by asking follow up questions and helping them with creating a realistic timeline. Allow the client to take a copy home for reference.
